Innovation and Opportunities to Address Social Determinants of Health in Medicaid Managed Care

In this report, authors explore the current innovations allowing Medicaid managed care organizations, in partnership with state agencies and local community organization, to better coordinate the health and social aspects of care to create better outcomes for individuals. the report also address opportunities to improve and expand this work, calling for better coordination and support for this work.
